Standard #: 22VAC40-185-40-K Description: Based on a review of written procedures, and an interview with administrative staff, the center failed to develop written playground safety procedures. Evidence: Administrative staff indicated that a playground safety plan was not written and provided to staff. Although staff have been told supervision procedures, the staff did not receive any written safety procedures for supervision, positioning of staff, use of the playground structure and maintenance of resilient surfacing. Plan of Correction: The program director will write the playground safety plan and send a copy to the licensing inspector. A copy of the written playground safety plan will be given to each staff member. All written policies will be provided to new staff during orientation training.
Standard #: 22VAC40-185-550-E Description: Based on a review of the emergency practice drills, and an interview with administrative staff, the center failed to maintain a record of the dates of the practice drills. Evidence: During the record review, staff member 1 could only locate the monthly evacuation drill documentation during the first half of the year, July 6, 2018 and on November 27, 2018. Staff member 1 indicated that the monthly drills were conducted in August, September and October and the individual pages used for each practice documentation could not be found. Plan of Correction: The program director will continue to search for the emergency drill records. A December practice will be conducted and recorded. The program director will establish a new and easy way to immediately file each emergency practice sheet so that the records are consistently stored in one place.
