Comments:
An unannounced monitoring inspection was completed on 12/26/18. There were 25 children present from nine months through eight years. Six children's records and five staff records were reviewed. No medications were present at the center. The inspector arrived at the center at 12:45 pm and departed at 4:30 pm. The children were observed during nap, during TV time after nap, during afternoon snack, and during free play times. Infant activities were also observed. There was discussion with the director about the following: the date on the fire inspection report form, documentation of lead teacher qualifications and having a lead staff person regularly present, practicing and documenting a second shelter-in-place for 2018, requirements for double ratio at nap and extra person present/available.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review and interview, the center failed to ensure that there was always a staff person on duty during the operational hours of the center who had obtained, within the last three years, instruction on performing the daily health observation of children. Evidence: 1. One staff person (staff #3) had documentation of daily health observation training in their staff record. Staff #3 was working 7:00 am to 4:30 pm on the date of the inspection and was not present for one hour during the inspection. The center's hours of operation are 6:30 am to 6:00 pm. 2. The director was asked if any other staff had the training. The director stated that no other staff had the training.

Plan of Correction: The director will plan a training for all staff.

Standard #: 22VAC40-185-260-B
Description: Based on record review and interview, the center failed to ensure that there was annual approval from the health department. Evidence: 1. The last food inspection and "Report of Environmental Sanitation Inspection" form that was completed by the health department official was dated 10/18/17. 2. The director was asked if there was a more current inspection and could not locate an inspection for 2018. Upon further investigation by the director by calling the health department, the director stated that the health department confirmed that the schedule for inspections was behind.

Plan of Correction: During the inspection, the director left a message with the food inspector to request an inspection.

Standard #: 22VAC40-185-370-5
Description: Based on observation and interview, the center failed to ensure that staff provided awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs. Evidence: 1. A nine month old infant was observed awake in a swing at the arrival of the inspector at 12:45pm. The infant was observed in the swing, still awake, at 1:45 pm. The infant was not provided a change in play space while awake at least every 30 minutes as the infant was in the swing while awake continuously for over one hour. 2. At 2:10 pm, the infant was observed awake and still in the swing. 3. The staff stated that the infant fell asleep some time after 2:10 pm and woke up by 2:40 pm.

Plan of Correction: At 2:40 pm, the infant was observed out of the swing on the floor.

Standard #: 22VAC40-185-420-E-1
Description: Based on observation of infant daily records, the center failed to ensure that all required information was documented for the infant in care. Evidence: 1. There was a bowel movement documented for the infant in care that occurred at 10:40 am. As of 2:45 pm, there was no documentation of the description of the bowel movement, as required.

Plan of Correction: The director will remind staff of the required documentation on the infant daily reports.
