Comments:
An unannounced Renewal Inspection was conducted on December 20, 2018, from approximately 10:15 am until 5:00 pm. At the time of entrance there were 64 children and 13 staff members present with children. There were additional staff on site for kitchen and administrative support. The sample size consisted of 6 children's records and 10 staff records. Two medications were reviewed. Children were observed during lunch and playing group games in the multipurpose room. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center program director during the exit interview. Please contact the licensing inspector with any question at 404-0568.

Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: On December 20, 2018, the transitional toddler were observed during their lunch time routine. Child #1 was observed sitting in a highchair and the tray had visible crumbs and dirt that moved when the Licensing Inspector touched them with a white napkin. The center staff confirmed the tray was visibly dirty and indicated they were told by another staff the trays had been cleaned.

Plan of Correction: The director will ensure the highchairs including trays will be deep cleaned. When new staff members enter the room they will check themselves to ensure trays will cleaned and sanitized before feeding.

Description: Based on observation and staff interviews, the licensee did not ensure that when food is brought from home all requirements are followed. Evidence: On December 20, 2018, in the transitional toddler room the following items were observed in the refrigerator and were insufficient. 1) One bottle was observed with milk brought from home as confirmed by center staff and the milk/bottle was dated 12-14-2018. The unused portion of opened food shall be discarded by the end of the day or returned to the parent. 2) Two bottles were observed and center staff confirmed it was milk brought from home and the bottles were not clearly dated and labeled in a way that identifies the owner.

Plan of Correction: The director will retrain the staff to check and make sure milk and food are labeled daily as required by licensing as it comes in. Retrain staff to send home food/milk at end of the day.

Standard #: 22VAC40-185-560-K
Description: Based on observation and staff interviews, the licensee did not ensure tables and high chairs/ trays shall be sanitized before and after each use for feeding. Evidence: On December 20, 2018, the transitional toddler children infants were observed during their lunch time routine. The center staff confirmed the highchair trays were not sanitized prior to the children eating. "Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: The director will ensure to retrain the staff that know proper sanitizing procedures and times for sanitizing.
