- Technical Assistance:
An unannounced monitoring inspection was conducted on December 20, 2018 from approximately 10:00 AM to 12:45 PM to determine the facility's compliance with licensing standards. The staff to child ratios were observed as follows: Infants 1:3, Toddlers 2:6, 2 year olds 2:5, 3 and 4 year olds 2:10, School Age 2:20. Lunch today consisted of a turkey sausage patty, english muffin, mixed vegetables, peaches, and milk. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The following required information was posted: the license, most recent inspection notice, emergency numbers, evacuation map/procedures, daily schedules, and various parent information. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. There were no medications at the center at the time of the inspection. Staff on site have current First Aid, CPR, and DHO training. Multiple staff have MAT certification. The most recent evacuation drill was documented on 12/13/18. The most recent shelter-in-place drill was documented on 10/23/18. Standard #: 22VAC40-185-130-A Description: Based on review of five children's records, two children's records did not contain documentation of immunizations. Evidence: The licensing inspector observed the records for Child #1 (start date 6/12/17) and Child #3 (start date 9/4/18) did not contain documentation of immunizations. Documentation of immunizations is required to be obtained prior to a child attending the center. Plan of Correction: The center will obtain the required documentation from parents.
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, two children's records did not contain documentation of a physical examination by or under the direction of a physician. Evidence: During the inspection, the licensing inspector observed the records for Child #1 (start date 6/12/17) and Child # 3 (start date 9/4/18) did not contain documentation of a physical exam. Documentation of a physical exam is required to be obtained prior to or within the first 30 days of a child attending the center. Plan of Correction: The center will obtain the required documentation from parents.
Standard #: 22VAC40-185-580-C Description: Based on observation, the center did not ensure that all required information was available in the transportation vehicles. Evidence: The licensing inspector observed that the two buses which were inspected did not contain the required emergency numbers or a document that includes frequent routes driven, potential shelters, hospitals, and evacuation routes. Plan of Correction: The center will ensure the information is updated in each bus.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of five staff records, the facility did not obtain results of a national fingerprint-based criminal history record check for three staff members in the required timeframes. Evidence: 1. The record for Staff #2 indicated a hire date of 5/1/02 and results from the national fingerprint-based criminal history record check dated 10/1/18. 2. The record for Staff #3 indicated a hire date of 8/24/09 and results from the national fingerprint-based criminal history record check dated 10/4/18. 3. The record for Staff #5 indicated a hire date of 2/28/02 and results from the national fingerprint-based criminal history record check dated 10/1/18. For staff employed prior to 1/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018. Plan of Correction: The center will ensure that timeframe requirements for background checks are met in the future.
