Comments:
An unannounced complaint investigation was initiated in the facility on December 20, 2018, regarding transportation. Interview with staff member and observation was conducted . Based on a interviews and observation during investigation, a preponderance of evidence does support the allegation, therefore the complaint is determined VALID.

Description: Based on observation, it was determined that any vehicle used by the center for the transportation of children shall the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of the children. Evidence: The bus , used for the daily transportation of children to and from the local public schools, had a Virginia safety inspection sticker that expired in September 2018. Staff #1 ( Director) confirmed that the bus had expired safety inspection sticker and that the annual safety inspection had not been completed for the bus.
