Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Friday, December 21, 2018 from 9:40am to 2:00pm. The children were observed engaged in a variety of activities ranging from playing in learning centers, playing on the playground and making holiday crafts. The center also conducted an emergency evacuation drill during the inspection. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/21/2018 Shelter-in-place drill: 07/04/2018 Last fire inspection: 10/08/2018 Last health inspection: 09/18/2018 Total enrollment today: 164 Today, the following child to staff ratios were observed: Infants (ages 6wk-12m), Ducklings: 9:3 Infants (ages 12-16m), Chipmunks: 7:2 Toddlers (ages 16-24m), Beavers: 7:2 Toddlers & Twos, Friendly Frogs: 7:3 Two-Three Year Room, Squirming Squirrels: 8:2 Three Year Room, Giggling Geese: 9:3 Three-Four Year Room, Rocking Rabbits: 19:2 Three-Four Year Room, Terrific Turtles: 15:3 Four-Five Year Room, Soaring Eagles: 12:3 Four-Five Year Room, Awesome Otters: 13:2 SA 1 (1st-2nd grade): 6:1 SA 2 (3rd-5th grade): 14:1 SA 3 (kindergarten): 12:1 SA 4 (2nd grade): 15:1 SA 5 (kindergarten): 11:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 12/21/2018, the center did not ensure that documentation of child updates and confirmation of up-to-date information was in each child's record. EVIDENCE: 1. The most recent update in the record of Child #6 is dated 06/19/2017. 2. The most recent update in the record of Child #9 is dated 06/20/2017.

Plan of Correction: Will email parents and remind them to return forms.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 12/21/2018, the center did not ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: An expired tube of Babyganics diaper rash cream was observed in the Frogs classroom. The cream expired on 03/2018.

Plan of Correction: Informed the teachers immediately. Will be on the agenda for the next staff meeting to look at the products expiration date, not just the authorization form.

Description: Based on a review of staff records on 12/21/2018, the center did not ensure that a central registry finding was obtained within 30 days of employment. EVIDENCE: 1. The central registry finding in the record of Staff #1, whose date of hire was 05/15/2018, is dated 07/24/2018. 2. The central registry finding in the record of Staff #4, whose date of hire was 07/03/2018, is dated 09/05/2018. 3. The central registry finding in the record of Staff #5, whose date of hire was 06/25/2018, is dated 10/01/2018.

Plan of Correction: Called to check on the status of the central registry findings, but didn't document conversations. Will begin to document phone calls with central registry.
