Comments:
An unannounced mandated Monitoring inspection was conducted today from 9:50am to 12:45pm. There were 20 children present with 5 caregivers. Children were observed painting, building with blocks, playing with cars, pretending to cook for their classmates, and playing outside on the playground. Reviewed 3 children's files, 4 staff files, emergency evacuation log, cribs and sleep mats, menu, health and fire inspections, playgrounds and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, each child did not have a physical examination within 30 days of their first day of care, Evidence: Child#1 (enrollment date - 8/13/18) did not have documentation of a physical exam.

Plan of Correction: I will ask the parent for another copy of the physical.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment outside is not being maintained in a safe condition. Evidence: The multi-colored Little Tykes dual slide on the Preschool Playground is leaning. Slides and leg supports have been compromised due to resilient surfacing pressure causing the structure to not be level, which is a hazard and safety concern.

Plan of Correction: I will put in a work order and have them pop off the slides and balance it.

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient surfacing does not comply with minimum safety standards. Evidence: Resilient material (mulch) has become compacted and therefore, does not meet minimum depth of 6 inches in fall zones.

Plan of Correction: I will put in a work order to have the mulch raked.
