63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on 12/17/2018 from 10:00 am to 1:30 pm. At the time of the inspection, 14 children were in care with 2 staff members present. The sample size consisted of 5 staff records and 5 children's records. Presently, there are no children in care at the center taking medication. Children were observed during rest, story time, hand washing, bathroom break, snack, transitions and behavioral guidance. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center failed to obtain all the required information for each child's record.
1. Out of the five children's records reviewed, child 2's record is the only record that contained the phone number for the parents' employers.
2. The director verified that they did not obtain the parents' work phone number.
Plan of Correction: The parent's work phone number will be made a required subject line on the application and enrollment online tools.
Standard #: 22VAC40-185-90--A Description: Based on record review and interview, the center failed to have parents sign a written agreement by the first day of the child's attendance that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease. Evidence: 1. Five children's records were reviewed and no written agreement pertaining to household communicable disease was found. 2. The parent handbook was reviewed and no policy was found regarding notifying the center of a household communicable disease. 3. The director was interviewed and she was unable to produce the required written agreement. Plan of Correction: The center requirement for parents to report students communicable disease will be updated to report anyone in the household's communicable disease.
Standard #: 22VAC40-185-420-E-3 Description: Based on interview, the center failed to provide at least semiannually in writing information to parents on their child's development, behavior, adjustment, and needs. Evidence: The director confirmed that the center does not provide written information on school age children's development, behavior, adjustment, and needs. The center does provide assessments for preschool aged students. Plan of Correction: A written summary of verbal assessments will be done during the next assessment process.
Standard #: 22VAC40-185-540-C Description: Based on review of the first aid kits and interview, the center failed to obtain all the required items for the first aid kit. Evidence: 1. The first aid kits in the preschool classroom on the upper level and the after care classroom on the lower level were found to contain only one triangular bandage in each kit when two are required to be in each. 2. The director verified that each first aid kit only contained one triangular bandage. Plan of Correction: Additional triangular bandages were ordered and will be placed in the first aid kits before students return to school from winter break.
Standard #: 22VAC40-185-540-E Description: Based on review of emergency supplies and interview, the center failed to obtain a working battery operated flashlight for the aftercare classroom. Evidence: 1. A search of the room was completed and no working battery operated flashlight was found. 2. The director verified that there was no flashlight in the room. Plan of Correction: Additional flashlights were ordered and will be placed in the first aid kit before students return to school.
