63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on 1/8/18 from 11:05am until 3:00pm. At the time of the tour, there were 76 children in care with 15 staff present. A sample of 8 children's records and 8 staff records were reviewed. Children were observed participating in morning learning activities, eating lunch and resting quietly during nap time. Infant diapering, hand washing, restroom and table sanitizing procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the center director during the exit interview.
Standard #: 22VAC40-185-130-B Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years. Evidence: 1. The most recent immunization record for child 1, age 1 year, is dated 4/30/18. Child 1 has an enrollment date of 4/23/18 and an updated immunization record was due in October 2018. 2. Administrative staff confirmed that updated immunization records for child 1 are overdue. Plan of Correction: An updated immunization record for child 1 will be requested from the parent and placed in the child's record once obtained by the center.
