Thank you for your time and assistance with the unannounced renewal inspection conducted today, January 8, 2019. The on-site inspection was from 11:00a.m. to 3:30p.m. On this date, there were 25 children present in three classrooms and in the care of six staff members. There were two additional staff members available to assist if needed. There were 42 children enrolled in the center. Five staff members' records and five children's records were reviewed. Three classrooms were toured and observations were made in two of those classrooms. Six staff members and three children were interviewed. There was no medication on-site or being administered, which is the policy of the center. Lunch was brought from home and afternoon snack met the USDA guidelines and consisted of vanilla wafers, applesauce and water. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The theme for the week was Dinosaurs and the children had enjoyed finding dinosaurs in frozen eggs. Some items reviewed/observed: transitions from the playground, bathroom and handwashing procedures, infant care, diapering changing procedures, lunch, story time, songs, nap time, afternoon snack, free play, updated liability insurance, annual health and fire inspections, first aid kit and emergency supplies, required posted information, diaper ointments and parent authorizations, injury reports, documentation of evacuation, shelter-in-place drills and lockdown drills, and updated emergency preparedness and injury prevention plans. Standard #: 22VAC40-185-240-C Description: Based on review of five staff members' records and verification from the director, two staff members failed to obtain 16 hours of required annual staff development training. Evidence: 1. Staff Member #2 obtained 9.5 annual training hours of the 16 hours required for 2017-2018. Date of hire for Staff Member #2 was 07/09/2016. 2. Staff Member #4 obtained 13 annual training hours of the 16 hours required for 2017-2018. Date of hire for Staff Member #2 was 02/22/2017. Plan of Correction: The director will ensure that both staff members complete the rest of their required training hours for the 2017 and 2018 year. Then they will complete 16 hours of training for the current year.
