Technical Assistance:
Technical assistance today included required postings, food service license, incident reports.

Comments:
An unannounced renewal inspection took place today from 11:30 am - 2 pm. There were four classes present in two buildings. I observed children eating lunch, listening to an interactive story, and a soothing preparation for naptime. Records were well organized. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some areas and equipment of the center not maintained in a safe condition. Evidence: There was a broken switchplate with a cracked surface covering an outlet in one room, and in another the broken prongs from a plastic safety cap accessible at child height in the outlet. There were some sharp areas on the edge of a radiator, and cords were dangling from cell phone chargers in some areas. An outside railing on the brick wall to the kitchen area was shakey. l

Plan of Correction: Areas will be covered with duct tape, and the outlet covers will be made inaccessible. We will talk to the church about the railing.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications today, one child did not have an authorization that met requirements. Evidence: The form available was an Asthma Action Plan from a physician which did not have a parent's authorization for the administration of medication.

Plan of Correction: I e-mailed the parent today and sent the correct form.

Standard #: 63.2(17)-1721.1-A
Description: Based on a review of record keeping today, there were no current Search of the Central Registry results or Sworn Disclosure Statements completed for two agents listed as an applicant on the renewal application. Evidence: Agent #1 and 2 had Registry Searches and Sworn Disclosure Statements that expired in 2012.

Plan of Correction: We will send the Central Registry Searches tomorrow, and have them complete Sworn Statements today or tomorrow.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on a review of records today, not all individuals listed in the business entity part of the application had fingerprinting done as required by 9/30/18. Evidence: There were no results from the Office for Background Investigations as required for Agent # 1 and 2.

Plan of Correction: They will get their fingerprinting done as soon as possible. I will send the results by next week.
