An Unannounced Monitoring Inspection was conducted on January 9, 2019 from approximately 10:25 am until 2:150 pm. Upon entrance there were 65 children and 11 staff present with children. Additional staff were on site for administrative and program purposes. The record review sample consisted of 7 children's records and 6 staff records and a review of background checks for 2 additional staff. Children were observed preparing for and eating lunch and during story time. The areas of non-compliance are noted on the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection on January 9, 2019, the following physical plant areas were noted on the playground. 1) On the younger children's playground the metal fence has one broken metal post at the top and there are two sharp edges. 2) On the older children's playground there is a wooden hand over hand climber and one of four of the supporting wooden post are splintered, cracked and deteriorating at the base. Plan of Correction: The center owner will not allow children to use the wooden climber until it replaced or repaired. The metal post will be covered until a fence company or repair man can fix it.
Standard #: 22VAC40-185-340-F Description: Based on observation and staff interviews, the licensee did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff, Evidence: During the inspection on January 9, 2019, at approximately 11:41 am the licensing inspector observed child #2 leaving the Three's classroom, walking to the center's kitchen while pushing the lunch cart and immediately returning to the Three's classroom. The kitchen is approximately 25 feet from the classroom and center staff confirmed child #2 was not being supervised during this time. The center staff did not ensure child #2 was always within site and sound supervision. Plan of Correction: Child #2 was the daily lunch helper and observed returning lunch items to the kitchen without sight supervision by staff. The center owner will retrain the staff and children will not longer be allowed to return items to the kitchen.
