Substantive changes to the subsidy vendor agreement were reviewed. A copy of the memo dated October 16, 2018 was left with the program director.
- Comments:

An unannounced subsidy health and safety inspection was conducted on 01/08/2018 from 11:45am - 2:15pm. A monitoring inspection was additionally conducted as this facility is a licensed child day center. At the time of entrance there were 38 children in care with 6 staff. The children ranged in age from infant - five years. Children were observed during lunch and nap. The sample size included eight children and three staff records.

Standard #: 22VAC40-665-540-A Description: Based on record review and interview, the vendor failed to ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1. There was no TB screening on file for staff 1 who began employment on 01.02.2019 and was working during the inspection. 2. Administrative staff confirmed that a TB screening was not on file for staff 1. Plan of Correction: Going forward all staff will obtain a TB screening prior to employment. Staff 1 will obtain a TB screening by 01/10/2019.
