Comments:
An unannounced Complaint Inspection was initiated in the center on December 21, 2018 , continued on December 28, 2018 and completed on January 10, 2019 regarding a complaint received by the department. Complainant reported concerns related to staffing /supervision. Based on review of center documentation and staff interviews, there was a preponderance of evidence to support the allegation of staffing/supervision, therefore the complaint has been determined to be VALID. There were violations found that were not reported and those areas of noncompliance are identified on the violation notice. Complaint related: Yes
Description: Based on interviews and documentation review, the licensee did not ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: On December 21, 2018, there was only one staff on site from approximately 6:30 am until 6:43 am with six children, age range of children was 17 months, (Child #3) to age 8 (Child #1). Child #1 and Child #2 arrived at 6:30 am. Child #3, Child #4, Child #5 and Child #6 arrived at 6:33 am. The second staff, Staff #2 arrived at the center at 6:43 am. Center documentation and interviews with parties involved confirmed this information.

Plan of Correction: The center director will have two staff members always in the morning and in closing.

Description: Based on interviews and documentation review, the licensee did not ensure that for children 16 months old to two years there was one staff member for every five children. Evidence: 1) On December 21, 2018, the child to staff ratio was insufficient from approximately 6:33 am until approximately 6:43 am. There were six children with only one staff member, Staff #1 and the age range of children was 17 months (Child #3 ) to age 8 (Child #1). Staff #2 arrived at the center at approximately 6:43 am. Center documentation and interviews with parties involved confirmed this information. 2) On December 28, 2018, the child to staff ratio was insufficient from approximately 9:40 am until 10:00 am. There were seventeen children with two staff members. The age range of children in care was 19 months (Child #7) to age 8, (Child #9). At approximately 10:00 am the group of seventeen children dived into two groups, a schoolage group and younger children's group. Also, at approximately 10:00 am, an additional staff member, Staff #3 began to assist with supervision of the younger children. Center documentation and interviews with parties involved confirmed this information.

Plan of Correction: The center director will hire more staff to make sure each class age is in ratio. Make sure staff is in compliance with ratio.

Description: Based on staff interviews and observation, the licensee did not ensure children three years of age or younger were not offered foods that are considered to be potential choking hazards. Evidence: During the inspection on December 21, 2018, two children, age two (Child #4 and Child #9) were observed during lunch eating bananas. The bananas were cut into circles and foods that are round, hard, small, slippery, or thick are considered high-risk foods for young children because they can lodge in small airways causing a child to choke.

Plan of Correction: The center director trained cook on choking hazards food and objects.
