Technical Assistance:

An unannounced renewal inspection was conducted on Monday, January 7, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 11:50 am and concluded the inspection at 2:41 pm. The census for today?s inspection consisted of 14 children in the care of 5 staff. Upon the inspector's arrival, the children were eating lunch. The children were later observed taking bathroom breaks and during rest time. Staff interacted with children in a positive manner. The classrooms are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Five children's records and five staff's records were reviewed during today's inspection. Violations from the previous inspection were reviewed for correction. Health Inspection: 03/01/2018 Fire Inspection: 02/28/2018 The last documented evacuation drill was held on 12/18/2018. A shelter-in-place drill was documented on 09/11/2018. A second shelter-in-place drill was documented on 03/15/2018. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-160-C
Description: Based on a review of five staff records on 01/07/2019, the center did not ensure that one staff resubmit tuberculosis (TB) screening results every two years from the date of the first initial screening or testing. EVIDENCE: 1) The record for Staff #2 (DOH: 06/14/2016) did not contain a current TB test/screening. Staff #2's last TB test/screening is dated 06/17/2016.

Plan of Correction: Per the Center: "Staff #2 was previously advised to update prior to expiration but did not receive follow-up. Staff member has been advised that suspension will be in effect until the TB screening is submitted."

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview on 01/07/2019, the center did not maintain a complete record for one child enrolled.

Evidence: 1) The first emergency contact listed in the record for Child #3 (DOE: 01/04/2019) did not contain a complete address. 2) The second emergency contact listed in the record for Child #3 did not contain a complete address. The state and city's zip code were missing from both emergency contacts. Each child's record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. 4) There was a name listed by the parent as not authorized to pick-up Child #3 and no accompanying legal paperwork. Administrative staff indicated the information was requested, but not submitted at the time of enrollment. Names of persons authorized to pick up the child and appropriate legal paperwork shall be on file when the custodial parent requests the center not to release the child to the other parent.

Plan of Correction: Per the Center: "New parent has been made aware that we still need a copy of any legal documentation that will prevent the child's father from being allowed to pick up the child. New parent has also completed the city and state for the emergency contact info."

Standard #: 22VAC40-185-270-A
Description: Based on observation at the center on 01/07/2019, the center did not ensure to keep all areas and equipment of the center in a clean, safe and operable condition. EVIDENCE: 1) In the upstairs School Age bathroom, the door's top hinge was not secured and when the door was opened, it would fall.

Plan of Correction: Per the Center: "The landlord has been made aware of this prior to inspection visit. The landlord was also informed that the delay caused a violation and has promised to fix the door this weekend."

Standard #: 22VAC40-185-320-B
Description: Based on observation at the center and interview on 01/07/2019, the center did not ensure the restroom used by the children in care was equipped with toilet paper, disposable towels or an air dryer within reach of children. EVIDENCE: 1) The Licensing Inspector observed the downstairs restroom did not have toilet paper or paper towels available for the children in care. 2) During interview, a staff member stated there was no toilet paper or paper towels stored in the downstairs restroom.

Plan of Correction: Per the Center: "A staff member has been assigned to ensure that if the clean up crew does not stock the bathroom said staff member must do it and check off on it each morning. Corrected during inspection."

Standard #: 22VAC40-185-560-N
Description: Based on observations on 01/07/2019, the center did not ensure children do not drink or eat while walking. EVIDENCE: 1) The Licensing Inspector observed two children walking while eating and drinking during lunch time.

Plan of Correction: Per the Center: "Disucssed this occurrence with the staff member responsible who apologized for this error."
