Background check regulations were discussed with the center director during the inspection.
An unannounced monitoring inspection was conducted on 1/10/19 from 11:45am until 2:15pm. At the time of the tour, there were 59 children in care with 9 staff present. Children were observed resting quietly during nap time and engaging in play with age appropriate toys. Infant feeding was also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, transportation, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child 1 contains no documentation of a physical examination. Child 1 had a date of enrollment of 11/19/18. 2. The center director confirmed that the record for child 1 does not contain documentation of a physical examination. Plan of Correction: Documentation of a physical examination for child 1 was obtained by the parent and placed in the child's record.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1. The most recent documentation of a tuberculosis screening for staff 1 is dated 9/30/16. 2. The center director confirmed that the tuberculosis screening for staff 1 is expired. Plan of Correction: Staff 1 obtained a TB screening on 1/10/19. Documentation of the screening has been placed in staff 1's record.
