Technical Assistance:
We discussed the requirements for staff records. There was no applicable risk assessment for one violation.

Comments:
Thank you for your assistance during the site inspection conducted from 10:00 AM to 12:10 PM and the staff record review conducted at the main office from 12:27 PM to 1:55 PM and from 3:10 PM to 4:00 PM. Today, there were twenty-five children with six staff and two volunteers. A family service worker was present to provide assistance. I viewed program activities, staffing, supervision, interactions with children, equipment, menu, lunch, posted information, medication, emergency supplies and drill records, five records for children, agency inspections, injury report, seven staff records, staff qualifications, staff training, indoor and outdoor areas. The children enjoyed a variety of classroom activity choices, table activities, stories, group discussions and outdoor play. The staff provided active supervision on the playground. Standard #: 22VAC40-185-160-B
Description: Based on a review of seven records for staff, and an interview with administrative staff, the tuberculosis screening results for one staff member did not include all acceptable forms of documentation. Evidence: A printed statement with the dates of TB screenings for staff member 5, including the latest date of May 11, 2018, did not include a signature by the physician or designated health care provider.

Plan of Correction: The administrator will make sure a statement with the most recent TB screening results is signed by the health care provider and a copy is placed in the staff record. Administrative staff who accept statements from new staff will make sure the required documentation was submitted for the record.
