Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection on Friday, January 11, 2019 from 9:45 AM until 3:10 PM. The following ratios were observed with a total of 71 children present during the inspection:
Infants - 12 children with 3 staff
Toddlers - 9 children with 2 staff
Twos (Discovery Preschool) - 17 children with 3 staff
Threes (Preschool) - 12 children with 2 staff
Fours & Fives (Pre-K) - 14 children with 2 staff
School Age - 9 children with 1 staff

The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, learning about transportation, creating silly stories using the letter ?Kk?, building with blocks, playing with toys, being held and rocked, eating lunch and resting at nap time. Children were also observed having their diapers changed, taking bathroom breaks and washing their hands. Lunch consisted of turkey sandwiches, celery and carrots, pears and milk. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, injury prevention plan, infant feeding schedules, daily attendance, daily sheets, practiced emergency drill logs, emergency preparedness plan, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication is administered and medication and medication authorizations were reviewed. Transportation is provided and the vehicles and transportation documentation were checked. Seven children's and seven staff records were reviewed

Standard #: 22VAC40-185-60-A
Description: Based on the review of seven children's records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #4 did not contain information regarding previous schools attended and policy regarding reporting communicable diseases. The record for Child #5 did not have one of the two required emergency contacts along with their required information.

Plan of Correction: We will request the form with the missing information. The second emergency contact was obtained during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observations, the center did not maintain all equipment in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed the following: 1. The wheels on two of the ride in cars located on the Twos playground were broken creating a pinching hazard. 2. The green sofa in the School Age classroom was torn and the foam was exposed.

Plan of Correction: The cars and the sofa will be removed by the end of today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed cleaning materials located in a cabinet in the Pre-K classroom. The cabinet contained a lock. The key was located in the lock.

Plan of Correction: Corrected during the inspection.
