Comments:
The licensing inspector conducted an unannounced inspection on Thursday, November 29, 2018 in response to a self-reported incident that the Department received on November 9, 20118 regarding special care provisions and emergencies. The licensing inspector was on site at the center on November 29, 2018 from 9:40 am to 11:40 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. There were violations cited during the inspection regarding special care provisions emergencies. Though not a part of the self-reported incident, there were violations cited related to background checks. See the violation notice for more details. Violations:
Standard #: 22VAC40-185-500-B
Description: Based on interviews with staff and review of documents, the center did not ensure when a child's clothing or diaper becomes wet or soiled, the the child shall be cleaned and change immediately. Evidence: During the inspection on November 29, 2018, the following were revealed during interviews with staff and review of documentation: 1. Interviews with Staff#1, Staff#2, Staff#3, and Staff#4 confirmed Child#1 had soiled clothing on November 8, 2018. According to Staff#1, Child#1 and their backpack was taken to the office and the staff returned to the classroom. 2. Staff#3 confirmed Child#1 was brought to the office with soiled clothing and the parent was contacted to change child's clothing as no staff were available. 3. A review of documentation revealed child remained in soiled clothing from approximately 15 minutes.

Plan of Correction: The childcare director will be available to take care of any thing staff are unable to do while in classroom.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records and interview with staff, the center did not ensure to obtain central registry findings for all staff within 30 days of employment or volunteer service. Evidence: During the inspection on November 29, 2018, in Staff#2's record (date of hire: 09/20/18) there was no findings of a central registry found. Interview with staff revealed a copy of the findings was not available during this inspection.

Plan of Correction: We will make sure to obtain central registry findings for all staff within 30 days of date of hire.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of staff record and interview with staff, the center did not ensure to obtain the Office of Background Investigation (OBI) fingerprinting determination letter were prior to staff beginning employment. Evidence: During the inspection on November 29, 2018, in Staff#2's record (date of hire: 09/20/18) the OBI fingerprinting determination letter was dated 09/28/18.

Plan of Correction: We will obtain OBI fingerprinting prior to all staff beginning employment.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records and interview with staff, the center did not ensure to obtain a copy of the results of an out of state central registry for all staff residing outside of Virginia within the last five years. Evidence: During the inspection on November 29, 2018, in Staff#2's (date of hire: 09/20/18) record it was documented that they lived outside of the state of Virginia. There was no documentation of results obtain for an out of state central registry for child abuse and neglect. Interview with staff confirmed there was no results of an out of state registry found during this inspection.

Plan of Correction: We will obtain a copy of central registry on staff who resided out of state within the last five years.
