During the inspection the licensing representative discussed with the Director: - daily health training requirements - physicals for recently enrolled children - inspector follow up needed with Fairfax Licensing Office Liason on updates for KCLC enrollment forms - ongoing improvements regarding program repairs and maintenance
An unannounced monitoring inspection was conducted today from 11:00am-2:00pm. A total of 43 children were in the care of 8 staff. Children were observed while engaged in various individual and group activities, handwashing, lunch [cheese pasta, pees, applesauce]. Infant bottle feeding and positive staff interactions were also observed. A sample of 4 staff records, 5 children's records, emergency drills and other documents were reviewed. Standard #: 22VAC40-185-130-A Description: Based on a sample of records reviewed, 1 out of 5 children's records did not include documentation of immunizations prior to the child's first date of attendance. Evidence: The record for Child #1 [date of enrollment: 1/7/2019] was missing an immunization record. Plan of Correction: We will request the information from the parents.
Standard #: 22VAC40-185-60-A Description: Based on a sample of records reviewed, 1 out of 5 children's records did not include the required information. Evidence: The record for Child #2 was missing the parent's employer and work phone number. Plan of Correction: We will request the information from the parent.
Standard #: 22VAC40-185-240-C Description: Based on a sample of records reviewed, documentation of 16 hours of annual training was not available at the time of the inspection for 3 out of 4 staff. Evidence: 1. Approximately 2.5 hours of training were documented for Staff #1 during the 2017-2018 program year. 2. 6 hours of training were documented for Staff #2 during the 2017-2018 program year. 3. Approximately 8.5 hours of training were documented for Staff #1 during the 2017-2018 program year. Plan of Correction: We are working to improve our training documentation for the current school year.
Standard #: 22VAC40-185-270-A Description: Based on observations, areas inside the center were not maintained in safe and operable conditions. Evidence: A loose floor tile and a chipped floor tile were observed in the two's bathroom - posing a tripping hazard. Plan of Correction: We received materials to have the floor repaired today.
