Today's unannounced focused monitoring inspection was completed on 01/15/2019 from 1:20 P.M.-4:45 P.M. The purpose of the inspection was for determining corrections to the previous violations and a self-reported incident that occurred at the center on 01/09/2019. At the time of the inspection there were 72 children in the care of seventeen staff and one administrator. The records for two children, eight staff, and documentation were reviewed, as well as two staff interviewed. The children were observed during infant feeding time, diapering, naptime, handwashing, and snack time. The exceptions to compliance are noted on the violation notice. There were no violations. Standard #: 22VAC40-185-340-A Description: Based on interview and review of center documentation, it was determined that the center failed to ensure that when staff are supervising children, they ensure the care, protection, and guidance. Evidence: 1. On 01/09/2019 at approximately 2:30 P.M., Staff #8 was preparing snack for the infants. She took Child #1's bottle from the 12:00 feeding that had approximately one ounce of breast milk remaining and added it to Child #2's bottle for snack, and then gave the bottle to Child #1 to drink. When Staff #8 went to go locate Child #2's snack bottle she could not locate it, but located a full bottle for Child #1. When Staff #8 realized what had occurred Child #1 had drank approximately one-two ounces of Child #2's breast milk. 2. Staff verified that Child #1 had consumed Child #2's breast milk, and that there had been a mistake made when preparing the snack bottles. Plan of Correction: The director met with the Infant Staff and reviewed all required standards regarding infant feeding. They also have set up a check list of steps that should occur when preparing formula and breast milk for all infants.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of staff records, it was determined that one staff member required an out of state central registry background search that was not completed. Evidence: 1. Staff #8 began employment on 03/26/2018. This staff person indicated that she resided in one state other than Virginia within the past 5 years. The required out of state central registry search request for this staff person has not been submitted. 2. Staff verified that the record check had not been submitted within the required time frame. Plan of Correction: The current program director will have the staff member fill out the required paperwork and send off for the Central Registry Background check immediately.
