Discussed time frame of acceptable TB test/screenings for new hires.

An unannounced renewal inspection was completed today. The physical plant, 5 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:30-3:30pm. There were 117 children under the supervision of 25 staff. During the inspection, children were observed enjoying free play, being fed a bottle (infant), reading books and resting during naptime. For lunch, the children were served sweet and sour meatballs, green beans, apples and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Standard #: 22VAC40-185-140-A Description: Based on file review, 1 out of 5 children's records does not have a physical examination. Evidence 1. Child #3 does not have a physical on file. Plan of Correction: The Director emailed the father during the inspection requesting that this be brought into the center.
Standard #: 22VAC40-185-160-A Description: Based on file review, 1 out of 5 staff records does not have a TB test/screening for a new employee completed within 12 months prior to employment. Evidence 1. Staff #4 began employment on 12/14/18. The TB test/screening on file was completed on 11/7/17. Plan of Correction: Staff #4 will have this completed again.
Standard #: 22VAC40-185-440-B Description: Based on observations, there were not enough cribs in the Infant classroom. Evidence 1. There were 12 infants being cared for in the classroom. However, there were only 10 cribs. When asked where they all sleep, teachers said that some of the children were part time and that some did not sleep very often. The LI informed the staff that each child must be assigned to their own crib. Plan of Correction: A crib from the Toddler classroom was brought into the class. Another crib will be ordered.
Standard #: 22VAC40-185-510-C Description: Based on observations, the center did not follow all procedure for administering medications. Evidence 1. Child #6 requires Levalbuterol to be administered on an as needed basis while in care. This medication expired on 8/18. Plan of Correction: This medication has not been administered since May 2018. The Director will speak to the mother about whether or not this is still needed at the center.
