Comments:
An unannounced monitoring visit and facility tour was conducted from 10:15am-11:35am. At the time of inspectors' arrival there were 14 preschool aged children in care with 3 staff members. Children were observed participating in instructional time, and transitioning between activities. A sample of 5 children's records and 4 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are detailed on the violation notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-240-C
Description: Based on record review an interview it was determined the center did not ensure that each staff member who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: 1. The record for staff #2(hired 8/1/16) only had 8 hours of accepted training documented for 2017-2018. 2. The Director confirmed that staff #2 did not have other training hours.

Plan of Correction: Staff will have 16 hours of accepted trainings annually.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: In the school aged room there was a tv on top of a 3' high bookcase that was not stable and wobbled when touched.

Plan of Correction: Director will have the shelf anchored.
