63.2(17) License & Registration Procedures
An unannounced monitoring inspection was completed on January 4, 2019 from approximately 10:45a.m. to 1:15p.m. Fifty-eight children were in care during the inspection. Staff and children, ages infant to school age, were observed in seven classrooms with adequate staff ratios. Children and staff were observed engaging in activities to include outside play time, activities for infants, preparing for nap time, lunch, diapering and toileting. Children?s equipment, learning materials, outdoor and classroom space, restrooms and the first aid and emergency supplies were inspected. Six children?s records, six staff records, annual fire and health inspections, fire drill, shelter in place and lock down drill logs, daily health observation and medication administration certifications, injury reports, daily attendance, the emergency preparedness plan, medication authorizations and logs and required postings were reviewed. The Director and Co-Director were available for interview and the inspection and were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-70-A Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of six staff. Evidence: 1. Staff #5 was hired on August 20, 2018 as program lead. 2. Staff #5's record lacked written information to demonstrate that Staff #5 possesses the education, training and/or experience required for the job position. 3. Administrator #1 acknowledged this information was not documented in the staff's record. Plan of Correction: The Director will have the staff provide the missing information for the record.
Standard #: 22VAC40-185-570-E Description: Based on observation, the licensee failed to ensure that prepared infant formula is dated and labeled with the child?s name. Evidence: 1. All the bottles of prepared infant formula were observed in the infant classroom. None of the bottles were labeled with a date. 2. Administrator #1 acknowledged the bottles were not labeled with the date. Plan of Correction: The Director will implement disciplinary action for the staff responsible for dating the bottles in the infant room.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, the licensee failed to obtain a child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years by the end of the 30th day of employment for one of six staff. Evidence: 1. Staff #4 was hired on May 22, 2017. 2. Staff #4's record indicated Staff #4 has resided in another state in the preceding five years. However, Staff #4's record did not contain an out of state child abuse and neglect registry search for the other state. 2. Administrator #2 advised this was completed but that the results were not placed in the staff's file. Plan of Correction: The Co-Director will complete another child abuse and neglect registry or equivalent registry maintained by the the state and will place the results in the staff's record. The Director will send documentation to the Licensing Inspector that the background checks have been requested no later than 10 business days following notification.
