63.2(17) License & Registration Procedures

Technical Assistance:
Sight and Sound Supervision for all children under the age of 10 years old.

Comments:
An unannounced monitoring inspection was conducted on January 16, 2019. Upon arrival to the center there were 14 children present with two staff members in the cafeteria having breakfast. The children were also observed during toileting, handwashing, having circle time, during music and movement and playing in centers. The licensing inspector reviewed the physical plant, administration, required postings and documentation, health and safety supplies and procedures, nutrition and supervision. Two staff records were reviewed. Five board members background checks were reviewed at the main office. Five children's records were reviewed. One medication was reviewed. Exit interview was conducted with the Program director/Lead Teacher and .

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on January 16, 2019, the fire inspection report for the center was more than one year old and was dated 5/18/17.

Plan of Correction: We will continue checking with the school to get a copy.

Standard #: 22VAC40-185-550-D
Description: Based on review and staff interview, the licensee did not ensure that the center shall implement a minimum of two shelter-in-place drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on January 16, 2019, the center had documentation of conducting only one shelter-in-place drill in 2018 (3/20/18).

Plan of Correction: We will make sure to have two for 2019.

Standard #: 22VAC40-191-60-B
Description: Based on record review, in one of seven staff records reviewed, the licensee did not ensure that an employee, volunteer or board member must not be employed or provide service until the agency has the person's completed sworn statement or affirmation. Evidence: During the inspection conducted on January 16, 2019, there was not a completed sworn statement or affirmation for Board Member #5.

Plan of Correction: Administrative Assistant sent an e-mail requesting the sworn statement for Board Member #5. The completed form will be faxed to the licensing inspector.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, in two of seven staff records reviewed, the licensee did not ensure that staff, board members and/or agents submit to fingerprinting and obtain results. Evidence: During the inspection conducted on January 16, 2019, there was no documentation of fingerprint results for Board Member #4 and Board Member #5.

Plan of Correction: An e-mail has been sent to those board members stating the instructions on how to complete the fingerprints. The results will be emailed to the licensing inspector.
