The provider?s responses for the ?Description of Action To Be Taken? were not received as of 01/16/19 and will not appear on this Violation Notice. The licensing inspector conducted an unannounced complaint inspection on 12/03/19 in response to a complaint that was received in the licensing office regarding allegations in the area of supervision. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegations, so the complaint is determined to be VALID.
Standard #: 22VAC40-185-350-E-2 Complaint related: No Description: Based on a record review and interviews the center did not ensure the required ratio of one staff for every 5 children was maintained in the 16 months to 2 years old room. Evidence: 1. On 11/14/18, the toddler room was out of ratio with 1 staff (Staff #1) from 7:33am - 8:30am. At 7:33am there were 6 children in care with the youngest child being 18 months. By 8:26am there were 10 children in care with 1 staff member (staff #1) until a second staff (staff #2) entered the classroom at 8:30am 2. On 11/16/18, the toddler room was out of ratio with 1 staff (Staff #1) from 8:15am - 8:30am. At 8:15am, there were 6 children in care in care with the youngest child being 18 months. By 8:22am there were 8 children in care with 1 staff member (staff #1) until a second staff (staff #2) entered the classroom at 8:30am. Plan of Correction: Not available online. Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on a record review and interviews the center did not ensure the written record of children's injuries included all required components. Evidence: 1. On 11/14/18 child #1 was picked-up from the center and his left eye was swollen and a scrape was present next to his eye. The Assistant Director and Staff #2 reported seeing the scrape on the child's eye, however the center did not have a written record of the injury. 2. On 11/16/18 staff #2 documented seeing an injury, which was described as looking like a "bite mark" to child #1's left hand. The incident report did not include the following: first aid treatment or action take, any future action to prevent recurrence of the injury/incident. Staff #1 who was present at the time the injury was seen by staff #2 was not listed on the incident report. Plan of Correction: Not available online. Contact Inspector for more information.
