63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on January 16, 2019 from approximately 9:45 am to 12:20 pm. Two staff were present with eleven children at the onset of inspection. Additional staff arrived during inspection. Five children's records and four staff records were reviewed.
Standard #: 22VAC40-185-140-A Description: Based on review, facility did not ensure 1 of 5 children's records has a physical examination within a month of beginning attendance. Evidence: On January 16, 2019, the record for child #5 (start date December 4, 2018) did not have a physical. Plan of Correction: The physical will be requested today.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review, facility did not ensure there is an out of state central registry check completed when a staff has lived outside the state of Virginia within the past five years. Evidence: On January 16, 2019, the following was found during record review of four staff records: 1. The record for staff #1 (start date November 12, 2018) had documentation of living outside of Virginia within the past five years and there was not a out of staff central registry check conducted for that state. 2. The record for staff #3 (start date June 5, 2018) had documentation of living outside of Virginia within the past five years and there was not a out of staff central registry check conducted for that state. Plan of Correction: The checks will be requested today.
