An unannounced monitoring inspection was conducted with the center director and center staff on January 17, 2019 to determine compliance with the licensing standards. There were 4 infants, 7 two year olds, 13 preschool children, and 11 school age children present with five staff actively supervising. The children were having rest time and waking for afternoon snack. Snack served today: animal crackers and apple juice. Seven staff files and four children's files were reviewed - see Supplemental Summary for missing items. The facility does not administer medication. The last health inspection was conducted 1/2/2019. The last fire inspection was conducted 5/11/2018. The last shelter in place drill was conducted 8/23/2018. Evacuation drills have been practiced monthly. The playground was not reviewed today due to inclement weather - snow cover. Standard #: 22VAC40-185-160-C Description: Based on review of seven staff files, the facility failed to have documentation of a follow up tuberculosis (TB/PPD) test or screening within two years from the date of the previous screening. Evidence: The tuberculosis results for Staff 1 expired 10/6/2018, Staff 3 expired 11/23/2018, and Staff 6 expired 11/22/2018. Plan of Correction: Staff will get an updated TB/PPD test as soon as appointments can be made.
