To find the Search for the Child Registry for other states (if an individual has lived in another state in the past 5 years), go to the website and look for background checks and the Adam Walsh Center.

An unannounced renewal inspection was conducted today from 10:45 pm - 1:30 pm. There were 18 children present with four-five staff. I observed children listening to a story, singing songs, and pouring over photos and discussing the travels of a plush "Gruffalo" who recently accompanied a family on a Hawaiian vacation. Description: Based on a sample of children's files today, there was no documentation of immunizations as required before attendance at the center. Evidence: Child # 3 and 5 did not have documentation available today to demonstrate adequate immunizations at the time of entry into the program (2/18 and 11/!8).

Plan of Correction: Parents will be contacted to bring this in.

Standard #: 22VAC40-185-140-A
Description: Based on a review of a sample of children's records today, there were no physical exams available for review for some children. Evidence: There were no physical exams documented for Child # 1, 2, 3, and 4. All children had been in care over 30 days.

Plan of Correction: Parents will be contacted to bring in the physicals.

Standard #: 22VAC40-185-40-D
Description: Based on observation, the license was not posted in a place conspicuous to the public per the Code of Virginia. Evidence: The license was posted behind a door in the room adjacent to the kitchen where it was not conspicuous to visitors.

Plan of Correction: Placed in front room during inspection.

Standard #: 22VAC40-185-40-J
Description: Based on a review of records today, there was no documentation of a review of incident reports as required. Evidence: Although incidents were documented, there was no documentation of a timely review on which to base future injury prevention procedures.

Plan of Correction: We will make a spread sheet to look for possible patterns.

Standard #: 22VAC40-185-60-A
Description: Based on a review of a sample of children's files, some required information was not documented. Evidence: Emergency contact information was not complete for 3 of five files reviewed. Two needed an additional emergency contact; two needed addresses of emergency contacts.

Plan of Correction: Parents will be contacted for this information.

Standard #: 22VAC40-185-240-C
Description: Based on a review of staff records today, some staff did not have documentation of at least 16 hours of annual staff training as required. Evidence: A review of four staff files found that Staff #2 and 3 had less than 16 hours of annual training.

Plan of Correction: We will make sure all staff have documentation of training.
