Discussed with center director classroom equipment.
An unannounced monitoring inspection was conducted from 11:10 am-1:00pm. 13 children were in care with 8 staff members present. Children were observed engaged in small group activities and eating lunch. The lunch menu included tuna salad sandwiches, applesauce, green beans and milk. 3 staff and 5 children's records, emergency drill records and medications were reviewed. Standard #: 22VAC40-185-160-A Description: Based on record review and staff interview, the center did not obtain documentation of a negative tuberculosis (TB) screening for each staff member and individual from an independent contractor. Evidence: 1. Staff # 1's record had a medical form indicating that the TB test was placed on 4/17/18. The results were not obtained to indicate that the test was negative. 2. Staff # 2 is an individual working with a child with special needs. Staff # 3 stated that Staff # 2 has been working with the child since before October 2018. Documentation of a negative tuberculosis screening was not obtained for Staff # 2 and was not available for review. Plan of Correction: We will have Staff # 1 call the doctor's office so they can send in the results. We have asked Staff # 2 to email us a copy of her TB Screening.
