Comments:
A Subsidy Health and Safety inspection was conducted on 1/15/19 to review supplemental health and safety requirements. There were 13 children with 8 staff members. The children were observed playing in small groups and eating lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on staff interview, the center did not implement written policy and procedures to ensure how each group of children receives care by consistent staff members. Evidence: Staff # 3 stated that the center has not yet implemented written policy and procedures to ensure group sizes for each group of children.

Plan of Correction: We will work on the group size policy and plan.

Standard #: 22VAC40-665-770-B-5
Description: Based on record review, continuity of operations procedures to ensure that essential functions are maintained during an emergency were not included in the emergency preparedness plans. Evidence: The center's emergency preparedness plan did not include the newly required procedures to show continuity of operations procedures during an emergency.

Plan of Correction: We will add the continuity procedures to our plan.
