An announced initial inspection was completed on 1/22/19. An initial application for a license was received by the Division of Licensing Programs on 10/10/18. The application was complete as of 11/19/18. All required policies and procedures were reviewed along with the owner and director's record. Two children's records were reviewed. The licensing protocol brochure was provided and the risk assessment matrix was reviewed. The owner requests a starting date for licensure for 1/28/19. The inspection began at 9:55 am and concluded at 1:30 pm.

Standard #: 22VAC40-185-40-I Description: Based on review of policies and procedures and interview, the center failed to ensure that there were written procedures for injury prevention. Evidence: 1. The policies and procedures were reviewed and no written procedures were observed for injury prevention. 2. The owner and director were interviewed and stated that there were no written procedures for injury prevention. Plan of Correction: The owner and director will develop procedures and add them to the written policies and procedures of the center.
Standard #: 22VAC40-185-550-B Description: Based on review of policies and procedures and interview, the center failed to ensure that all required components were a part of the emergency preparedness plan. Evidence: 1. All policies and procedures were reviewed. The following procedural components were not found in the center's emergency preparedness plan: availability and primary use of communication tools, method of communication after evacuation and shelter-in-place, staff training requirement, plan review and update. Plan of Correction: The owner will complete the model form for emergency preparedness.
