Technical Assistance:
On the toddler playground, a small composite structure had turf underneath it. Due to previous pea gravel used as surfacing, it has mixed in on the turf. Keep in mind to ensure the resiliency a playground surface, it is not recommend materials mix.

Comments:
An unannounced monitoring inspection was conducted from 9:20am - 11:30am. Upon arrival there were 71 children in care with 16 staff present. A sample of 4 staff and 7 children's records were reviewed in addition to 1 medication. Children were observed outside on the playground, playing in learning centers and during circle time. Infants were observed sleeping in cribs. The lunch menu for the day was grilled chicken nuggets, oranges, mixed vegetables, wheat bread and milk.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. In the two year-old room one of the two toilets did not have a tank cover. The plumbing and water was exposed. 2. In the 4 year-old room the metal hardware attached to the toilet plumbing was rusted. When the inspector wiped it with a tissue, a reddish brown substance came off. 3. The closed hooks on the toddler swings were rusted.

Plan of Correction: 1. This was corrected 1/18/19 2. This was corrected 1/18/19 3. This will be completed by 2/17/2019 but the hooks have been removed in the interim. The safety captain will be doing daily walk throughs to make sure compliance and report deficiencies within 24 hours for management to handle. Management will also periodically inspect for compliance.

Standard #: 22VAC40-185-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: The toddler swing set did not have any resilient surfacing underneath the set or in the fall zones.

Plan of Correction: This will be completed by 2/17/19 The safety captain will be doing daily playground checks to make sure compliance and report deficiencies within 24 hours for management to handle. Management will also do periodic walkthroughs to ensure compliance.
