Discussed process of out of state central registry requests and timeframes.
An unannounced Renewal inspection was conducted this date. A total of 44 children with 7 staff present. All 7 classrooms were observed. The children were busy playing with legos, drawing and painting birds on branches. Artwork displayed throughout the center that consisted of snowmen, identifying parts of a flower, graphing favorite foods and practicing writing by listing a grocery list. The center was well equipped with ample supplies and equipment age appropriate for the children in care. A total of 5 children's records and 4 staff records were reviewed. Standard #: 22VAC40-185-280-B Description: Based on observation of the Space and Flight classroom, a hazardous substance was found unlocked. Evidence: A spray bottle of bleach and water was found in a cabinet. The cabinet was found unlocked. Plan of Correction: We will secure this immediately.
Standard #: 22VAC40-185-500-B Description: Based on observation of the Foxhounds Class, diapering procedures were not properly followed. Evidence: The one staff in the Foxhounds Class was observed changing one child's diaper. Staff failed to wash hands directly after changing the child, was observed placing a cap on a plastic receptacle and then proceeded to wash their hands. This same staff was observed disposing the soiled diaper by lifting the lid of the step on receptacle with their hands, rather than using the step on procedure in order to prevent contamination. This was observed did not wash and sanitize the diaper surface after changing the child. Plan of Correction: We will require this staff to train on diapering procedures and will continue to observe the diapering.
Standard #: 22VAC40-185-520-C Description: Based on observation of the Butterflies classroom, one diaper ointment did not meet all requirements. Evidence: One diaper ointment (A and D ointment) was found in this classroom not labeled with a child's name. Plan of Correction: We will label this ointment today.
Standard #: 22VAC40-185-560-I Description: Based on observation of the Foxhounds classroom, food was not stored in a sanitary manner. Evidence: Two beverage containers were found in children's cubbies containing milk. The milk was maintained at room temperature. Plan of Correction: The teachers will require the food brought home to be stored with an ice pack or be refrigerated. If the ice pack is not brought, the food will be removed from the classroom.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, out of state background checks were not requested. Evidence: Staff #1's out of state central registry request was returned due to lack of payment and submitting an incorrect request form. Follow up for this request was not conducted. Staff #2 disclosed on the Sworn Statement that they have lived out of state in the past 5 years. No out of state central registry request was made. Plan of Correction: We will require both staff to complete the requests by this Friday and will follow up with staff within 45 days of the request and document the follow up.
