The licensing inspector conducted an unannounced monitoring inspection on 01/17/2019 from 10:03am to 2:20pm. The children were observed playing in centers, reading books, participating in story time, playing on the playground and eating lunch. A formal observation was completed with the wobbler and two year old classrooms. Lunch consisted of beef patty, bread, mashed potatoes, green beans and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as two mini buses. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Five injury reports were reviewed as well as the center?s injury prevention plan, which was updated 01/10/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/10/2019 Last shelter-in-place drill: 11/30/2018 Last fire inspection: 08/16/2018 Last health inspection: 09/13/2018 Today, the following child to staff ratios were observed: Infants - 12:3 Woddlers ? 3:1 Twos (A) - 12:2 Twos (B) - 8:2 Threes and Fours - 14:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
Evidence:
1. The two year old (B) classroom had an uncovered outlet located on the wall.
2. The toddler classroom had three uncovered surge protector outlets located on a counter, out of the reach of children.Plan of Correction: Corrected during the inspection.
