Comments:
A Subsidy Health and Safety inspection was conducted today to review supplemental health and safety requirements. There were 51 children in care, with a total of 6 staff members assigned to their direct supervision. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on a review of administrative documents, it was determined that one required allergy document is not on site. Evidence: There is no list of children's food allergies, sensitivities, and dietary restrictions.

Plan of Correction: I will print the list and update all staff on the information.

Standard #: 22VAC40-665-520-B
Description: Based on a review of children's records, it was determined that the children's food allergy action plans have not been completed as required. Evidence: The food allergy action plans have not been signed/dated by a physician.

Plan of Correction: The parents will be asked to have the form signed.
