An unannounced monitoring inspection was conducted on January 11, 2019 with the center director and center staff to determine compliance with the licensing standards. There were 5 infant and toddlers with 2 staff, 15 preschool children with two staff, and four school age children with one staff. Ten additional school age children arrived after school. The children were having rest time and eating afternoon snack. Snack served today: pirates booty and fruit snacks. Ten staff files and five children's files were reviewed - please see Supplemental Summary for missing items. The last fire inspection was conducted 4/13/2018. The last health inspection was conducted 5/9/2018. The last shelter in place drill was conducted 9/17/2018. Standard #: 22VAC40-185-160-C Description: Based on review of ten staff files, the facility failed to have documentation of a repeat tuberculosis test or screening within two years from the date of the initial screening or testing. Standard #: 22VAC40-185-510-C Description: Based on review of five medications for five children, the facility failed to have a long term medication authorization, signed by the child's physician, on file for each medication that was for a longer duration than ten work days. Evidence: the short term medication authorization for Child 2 was completed by the parent on 12/18/2018. The short term medication authorization for Child 4 was completed by the parent on 11/5/2018.
