Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 11:00 am - 12:30 pm. There were 22 children in care being supervised by 3 staff members. The children were observed preparing for and eating lunch as well as participating in table top activities, and cooperative games. An allergy list was observed for children attending the program with diagnosed food allergies. Allergy care plans were reviewed for those children with diagnosed food allergies. Staff training records were reviewed for completion of CPR, First Aid, and Virginia Pre-Service training. The most recent Lock Down drill was conducted 01/17/2019. Description: Based on review of the allergy list and review of allergy care plans it was determined that not all children with a diagnosed food allergy had a written allergy care plan completed by a physician on file. Evidence: The allergy list indicated that Child A had a diagnosed food allergy however, there was no written allergy care plan on file for child A.

Plan of Correction: Administration will request a written allergy care plan to be completed by the physician from the parents of Child A.
