An unannounced non-mandated inspection was conducted this date. The purpose of this inspection was to evaluate the corrective actions provided in the terms of the Intensive Plan of Correction (IPOC) and the Special Order which was signed on January 8, 2019. The center has implemented some trainings and monitoring systems over the past four weeks. Observation of programming, review of mandatory staff training curriculum and verification of staff attendance was made. Two new staff records were reviewed that were hired in the past 3 months. Standard #: 22VAC40-185-70-A Description: Based on staff record review, references were not obtained. Evidence: Staff #1 was hired on 1/22/19. No documented references were found in their record. Plan of Correction: Director obtained two references for this staff during the inspection.
Standard #: 22VAC40-191-60-C-2 Description: Based on staff record review, one background check was not obtained within 30 days of employment. Evidence: Staff #2 was hired on 11/15/2018. No documentation found that the Central Registry search result was received. Plan of Correction: We will contact the central registry today and request the finding be emailed. Results will be forwarded to the licensing inspector.
