63.2(17) License & Registration Procedures

Comments:
Licensing inspector arrived at 1:47 pm and departed at 4:26 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were 56 children in care with 7 staff . Children were observed during nap time and arriving from school . The sample size consisted of 5 children records and 6 staff records. Areas of non-compliance was reviewed with the director.

Standard #: 22VAC40-185-240-D-4
Description: Based on staff interview and observation, the center did not ensure there was a MAT trained staff present when a child is in care whom have been prescribed medication. Evidence: On January 17, 2019, upon arrival to the center there was not a staff present for approximately 10 min that was MAT trained and children emergency medication was at the center . Confirmed with staff.

Standard #: 22VAC40-185-340-B
Description: Based on record review, the center did not ensure during the center hour's of operation, one adult on the premises was in charge of the administration of the center. Evidence: On January 17, 2018, upon arrival the staff member was unable to find the staff #1 . Staff #1 was designated to be in charge on the premises. Staff confirmed staff #1 was not present at the center. Staff #1 was next store. Staff #1 returned to center approximately 10 minutes after my arrival to the center.

Standard #: 22VAC40-185-440-J
Description: Based on observation , the center did not ensure there was thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: On January 17, 2019, there was one occupied crib with baby seats on both sides of the occupied crib. There was not a service side.

Description: Based on 1 of 6 records reviewed and interview with program director, licensee did not ensure an Office of Background Investigation fingerprinting determination letter prior to staff beginning employment. Evidence: During a inspection on January 17, 2019, staff # 1's fingerprinting determination letter was dated October 31, 2018 and orientation was conducted October 5, 2018. Staff employed before fingerprinting determination letter was received.
