Technical Assistance:
Technical assistance today included medication when transporting; physical environment and record keeping. We also discussed updating the training and names listed on emergency procedures as documented in the health binder.

Comments:
An unannounced monitoring inspection was conducted today from 12 - 3:30 pm. There were 53 children present. Ratios were maintained. I observed children busy with creative activities including pasting and painting, singing and preparing for lunch.Outdoor areas were not inspected due to weather. Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records today, some information was missing. Evidence: There were no documents as required to prove qualifications for the position of lead staff for Staff # 1.

Plan of Correction: We will get the records.

Standard #: 22VAC40-185-270-A
Description: Based on observation and staff confirmation during today's inspection, there were some areas and equipment of the center observed to not be maintained in a safe and operable condition. Evidence: There was peeling paint observed on the walls in the infant, toddler and three year old classrooms. There was adoor broken off a diaper table in one room. A large standing roll of paper was accessible in the children's area of one room, posing a falling hazard.

Plan of Correction: Work orders have been put in . It will be covered to prevent access by children. The paepr will be put in a closet today.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications today, one authorization did not give the staff permission to administer medication. Evidence: The paperwork for an authorization for Child # 1 gave permission for another program to administer the medication.

Plan of Correction: We will give it back to the parent today and request a new form that authorizes our program to administer the medication.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records today, not all written records of minor injuries included all the required information. Evidence: A sample of incident reports between 9/7 - 9/12/18 were observed to be missing date and time the parent was notified, and how the parent was notified.

Plan of Correction: We will be doing an incident report training by Monday.
