Technical Assistance:
The licensing inspector reviewed with the center's director the changes of subsidy inspection requirements effective 10/17/2018.

Comments:
A Subsidy Health and Safety inspection was conducted on Tuesday, January 29, 2019 from approximately 9:50 am to 2:00 pm review supplemental health and safety requirements. During this inspection, there were 32 children present with 11 staff including administrative staff. The licensing inspector observed the children in care participating in a variety of activities to include completing a coloring baker's cake worksheet, eating snack, engaged in circle time, tummy time, free play, and engaged in play with peers and staff at outdoor playground area. The children's snack today consisted yogurt and apple juice. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were reviewed by the inspector. During this inspection, there were six children's records and five staff records reviewed. The center's outdoor play area, bathrooms, and classroom were inspected. The school aged children were not present during the inspection but their designated areas were inspected. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. New subsidy requirements became effective on October 17, 2018. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. Standard #: 22VAC40-665-540-A
Description: Based on review of staff records and interview with staff, the center did not ensure all staff's tuberculosis screening was submitted at the time of employment and prior to coming into contact with children. Evidence: During the inspection on January 29, 2019, in Staff#4's (date of hire: 01/04/19) record a tuberculosis screening was not found. Interview with staff confirmed a tuberculosis screening not being completed and staff having contact with children.

Plan of Correction: The staff member will complete her tuberculosis screening tomorrow, January 30, 2019.

Standard #: 22VAC40-665-580-B
Description: Based on review of staff records and interview with staff, the center did not all staff complete Virginia Preservice Training of Child care Staff within the first 90 days of employment. Evidence: During the inspection on January 29, 2019, in Staff#1's record (date of hire: 8/24/17) there was no completion certificate found for the Virginia Preservice Training. Interview with staff confirmed a certificate of completion was not available.

Plan of Correction: The staff previously completed this training November 2017 but certificate could not be located. Staff member will redo training .

Description: Based on review of center's emergency response plan and interview with staff, the center did not ensure to include a continuity of operations to ensure that essential functions are maintained during an emergency. Evidence: During the inspection on January 29, 2019, the center's emergency response plan did not include procedures for continuity of operations. Interview with staff confirmed the required procedures for the continuity of operations was not included in procedures for emergencies at this time.

Plan of Correction: The procedures of continuity has been add to the emergency response plan during the inspection.
