63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted from approximately 7:30am-9:15am on January 30, 2019. Upon arrival to the facility, there were four staff present with 18 school age children in the gymnasium and cafeteria. Children were observed arriving to the program, coloring, playing cards and board games and talking freely amongst themselves. Five children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor areas, injury report documentation and injury prevention procedures, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation and procedures, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Four medications were reviewed in the center on this date. Exit interview conducted with the Recreation Supervisor.
Standard #: 22VAC40-185-510-C Description: Based on review and inspection of the facility, the center did not ensure that medication authorization include the duration date and that the center followed their outdated medication policy. Evidence: During the inspection conducted on January 30, 2019, the following was observed regarding medication observed at the center: 1. There were two medication authorization forms without a duration date. 2. There were two expired medications observed in the medication box. The center's policy states the following: All medications must be checked for expiration monthly. The center is to notify parents that medications are soon expiring 14 days prior to expiration. The center is to complete a Medication Return slip and return the medication to the parent. If the medication is not returned to the parent, the center is to complete a medication disposal slip prior to disposal. It further states that outdated medications should not be left at site locations. Plan of Correction: Recreation Supervisor will contact the parents to get the duration dates updated and to have the medication picked up and replaced.
