Comments:
An unannounced monitoring inspection was conducted on January 30, 2019. Upon arrival to the facility, there were 34 children present with five staff members in two classrooms. Observations included the physical plant, nutrition, supervision, staff/child ratios, health and safety, and children's records. Five children's records were reviewed. There were no medications present at the center. The children were observed having breakfast which consisted of french toast, apples and milk. They were also observed brushing their teeth, washing their hands, during circle and center time, having music and movement and listening during storytime. Classroom 524 was preparing to have parent volunteers participate in helping the children with three scientific gravity experiments.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's record maintained at the center was documented with all required information. Evidence: During the inspection conducted on January 30, 2019, the record for Child #4 did not include documentation of both parent's place of employment and the work phone number for each parent.

Plan of Correction: We will follow up with family service and make sure that we double check the files to make sure that all spaces are filled in.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: During the inspection conducted on January 30, 2019, in Room 525, there was a bottle of hand sanitizer on an open shelf that was labeled warning and keep out of reach of children.

Plan of Correction: The item was removed during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation and inspection of the facility, the staff members did not ensure that in areas used by preschool age children, electrical outlets have protective covers. Evidence: During the inspection conducted on January 30, 2019, in Room 524, there was an uncovered electrical outlet beneath the bulletin board that was accessible to the children in care.

Plan of Correction: We will double check outlets each morning to make sure they are all covered.
