- Technical Assistance:
Consultation was given of fingerprints results/e-mail needing to be returned to the center prior to staff working. Reviewed VA DSS web site for training opportunities, discussed drills as well as diapering sanitation methods.
An unannounced monitoring visit was conducted today from 10"30-12:15. Ten children were present with three staff which is within ratios for supervision. OBserved children preparing for snack (pretzel chips and apple juice, music time with a special visitor and free play. Physical space, emergency preparation, trainings and records for children and staff were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-160-A Description: Based on center management statement, a music contractor does not have documentation of a negative tuberculosis screening. Evidence: A music teacher was on-site today and no record of a TB screening is available. Plan of Correction: Music contractor owner will be contacted to provide documentation of the TB test results and will have it emailed to the center.
Standard #: 22VAC40-185-60-A Description: Based on record review, one of two children's records is incomplete. Evidence: Child #2 is missing two emergency contacts. Plan of Correction: Will contact parent to complete an emergency card.
Standard #: 22VAC40-185-70-A Description: Based on record review, one of three staff records is incomplete. Evidence: Staff #1 does not have a listed emergency contact Plan of Correction: Will complete paperwork and place in file.
Standard #: 22VAC40-185-240-D-6 Description: Based on staff statement, no one is present with current Daily Health Observation (DHO) certification. Evidence: DHO has expired for staff #3. Plan of Correction: Director will retake the on-line class which will be completed by Friday.
Standard #: 22VAC40-185-280-B Description: Based on observation, a cleaner is not stored in a locked place. Evidence: A container of clorox wipes is sitting on a window ledge is unlocked in the Pre-K classroom. Plan of Correction: Will be moved today.
Standard #: 22VAC40-185-290-3 Description: Based on observation, some electrical outlets did not have protective covers. Outlet covers were missing for 3 plugs. Plan of Correction: Outlet covers will be purchased today and replaced tomorrow.
Standard #: 22VAC40-185-510-N Description: Based on observation, an emergency medication was not returned to a family after the child no longer attended the program. Evidence: A prescription emergency medication was on-site and the child was no longer enrolled. Plan of Correction: Will contact the parent to pick-up within 14 days or dispose of the medication.
Standard #: 22VAC40-185-540-C Description: Based on review, a first aid kit is incomplete. Evidence: A second triangular bandage was not available. Plan of Correction: Will be purchased today and replaced in the kit tomorrow.
Standard #: 22VAC40-185-550-D Description: Based on record review and center statement, required evacuation drills were not practiced monthly. Evidence: A drill was not conducted for 9/2018. Based on record review and center statement, required shelter-in place drills were not practiced twice a year. Evidence: Two drills were not conducted for 2018. Plan of Correction: Moving forward two shelter drills will be conducted-one this week and monthly evacuation drills will be done as well.
Standard #: 22VAC40-185-560-G Description: Based on observation, a sample of lunch boxes were not dates and labeled with the child 's name. Evidence: Two were missing dates and names. Plan of Correction: Will remind parents to label containers with the child's name and date.
