An unannounced monitoring inspection was conducted today 1/29/2019 beginning at 6;20 A.M. and ending at 7:30 am. There were 28 children present today with five staff. The inspector reviewed five children's and five staff's records during the inspection. The children were observed during: arrival, free play, and game time. Discussion with site director about: sunscreen authorization form, injury form with parent method of notification and time of notification.
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, the center failed to ensure that all children had a physical within 30 days of enrollment. Evidence 1. Child # 3 did not have a physical exam within the 30 days of enrollment. The start date is 8/13/2018. Plan of Correction: The director will obtain a copy for the office at the school.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the center failed to ensure that all records were complete. Evidence: 1. Child # 2 was missing one the of the two emergency contacts. Plan of Correction: The director will have the parent to obtain the missing contact ASAP.
