Comments:
The licensing inspector conducted an unannounced renewal inspection Tuesday, January 22 , 2019 from 12:30pm to 3:20pm. The infants and toddlers were observed resting and being fed. The preschool children were observed resting on cots. All classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/26/2018 Last shelter-in-place drill: 09/08/2018 Total enrollment today: 54 Today, the following child to staff ratios were observed: Infants and Toddlers?10:3 Twos ?15:2 Threes ?19:2 Pre-K-10:1 School Age ?present The violations from the previous inspections were checked for corrections. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation on 01/22/2019, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. EVIDENCE: Three out of three depth samples taken of the mulch on the preschool and school-age playground did not measure the minimum depth of six inches.

Plan of Correction: Will order mulch to be delivered in two weeks. Will post signs stating that the children should not play on the equipment until the mulch is down.
