Comments:
The licensing inspector conducted an unannounced renewal inspection Thursday, January 24, 2019 from 1:00pm to 2:46pm. The children were observed resting on cots and mats. All classes, as well as the facility's van were inspected today. Five children?s records and five employee record were reviewed during this inspection. Medication is not being administered. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/31/2018 Shelter-in-place drills: 12/31/2018 Last fire inspection: 07/09/2018 Last health inspection: 08/22/2018 Total enrollment today: 35 Today, the following child to staff ratios were observed: Infants & Toddlers ?4:1 Twos ?10:1 (rest time ratio) Preschool ?11:1 (rest time ratio) School Age-not present Violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records on 06/22/2018, the center did not ensure that documentation that each child has received the immunizations were obtained before the child can attend the center. EVIDENCE: The record of Child #3,who started at the center on 10/15/2018, did not contain documentation of immunizations.

Plan of Correction: I know I have his immunization. I believe it was misfiled.

Standard #: 22VAC40-185-550-H
Description: Based on a review of documentation on 01/24/2019, the center did not ensure that the emergency documents kept in the vehicle that is used to transport children to and from the center contained the required information. EVIDENCE: The emergency document on the van used to transport children did not contain: local emergency contact information, information for potential shelters, hospitals, evacuation routes, and routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.).

Plan of Correction: Will have document completed by tomorrow. Will do a weekly check to make sure it is on the van.
