Technical Assistance:
Reviewed the Child Care Program Regulation (22VAC40-665) - Subsidy inspection requirements memo and Group Size Requirements Information Sheet that was distributed to subsidy providers on 10/16/2018.

Comments:
The licensing inspector conducted an unannounced subsidy health and safety inspection Wednesday, January 23, 2019 from 11:00am to 3:00pm to review supplemental health and safety requirements. The children were observed eating lunch, playing outside and completing an art project. They were later observed getting ready to rest on cots. The children bring their lunch from home. All classrooms and playgrounds were inspected today. The facilities mini buses were inspected today as well. Seven children?s records and seven employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Last fire drill: 01/07/2019 Last shelter in place drill: 11/20/2018 Lockdown drill: 11/20/2018 Last health inspection: 07/25/2018 Last fire inspection: 09/27/2018 Total enrollment today: 75 Today, the following child to staff ratios were observed: Infant and toddlers ?10:3 Twos I?10:2 Twos II-8:1 Threes I- 10:1 Threes II- 10:1 Fours- 27:3 School Age I ?not present A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on a review of staff records on 01/23/2019, the center did not ensure that documentation of the communicable tuberculosis (TB) screening shall be submitted at the time of employment and prior to coming into contact with children. EVIDENCE: The tb screening in the record of Staff # 2, whose date of hire was 01/04/2019, is dated 01/17/2019.

Plan of Correction: Will make a list of when things are due to keep track of time frames.

Standard #: 22VAC40-665-580-B
Description: Based on a review of staff records on 01/23/2019, the center did not ensure that within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff. EVIDENCE: The completion date for the Virginia Preservice Training for Child Care Staff for Staff #5, whose date of hire was 01/22/2018, is 07/18/2018.
