Comments:
The following Monitoring Inspection took place between the hours of 9:15am-1:00pm. At the start of this inspection there were a total of 128 children in the direct supervision of 26 staff members. A sample of 10 children?s records and 5 staff records were reviewed. The children were engaged in the following: playing board games, playing with blocks, coloring, putting together puzzles, diapering/toileting, and other organized activities. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, immunizations were not obtained for each child prior to the child attending the center. Evidence: 1. Child #3 did not have a complete immunization record on file and the child started on 1/25/2019.

Plan of Correction: We will ask the parents for this.

Standard #: 22VAC40-185-160-C
Description: Based on record review, staff did not obtain and submit the results of a follow-up TB screening. Evidence: 1. Staff #4 had a TB test dated 11/11/2016 which expired on 11//11/2018.

Plan of Correction: I will get a new one.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain a complete record for each child enrolled. Evidence: 1. Child #8 did not have the father's work information and phone number listed. 2. Child #5 started on 3/20/2017 and did not have documentation of an annual update of information. 3. Child #6 started on 2/27/2017 and did not have documentation of an annual update of information. 4. Child #7 started on 11/9/2015 and did not have documentation of an annual update of information.

Plan of Correction: We will ask the parents for this information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center were not maintained in a clean, safe, and operable condition. Evidence: 1. In the Toddlers classroom, there were multiple areas with chipped paint behind the bookshelf and the children's play sink. 2. In the Pre-K classroom, there were multiple areas of chipped paint above the calendar.

Plan of Correction: We will cover them until we get the walls painted.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets did not have protective covers. Evidence: 1. In the Toddlers classroom, there was one outlet without a cover. 2. In the Preschool classroom, there were two outlets without a cover behind a shelf.

Description: Based on observation, the most recent inspection of the facility was not posted. Evidence: 1. The inspection that was posted was dated 8/8/2017. The facility's most recent inspection was on 8/20/2018.
